The information on this website is intended to provide a thorough and comprehensive look at the disease, our resources, and our staff. In this section, you will find practical information, links that will help prepare you for your visit in advance, and travel and accommodation recommendations for our out of town patients. Don’t hesitate to call for additional details.
For our new patients, please print the required forms (links below) and bring a completed copy of each with you to your first visit. We also ask that you fax your records before hand (212-326-5725) or bring copies of your medical record the day of your visit, which should include recent:
- physician notes
- imaging studies
- pathology records
- blood work
- medication list (please see our list of forms)
If you’re not sure of the relevance of any record, bring it anyway.
We also ask that you begin the process of getting your pathology sent to us, as not delay the work-up of your case. Call the facility that performed your biopsy and ask them to send 10 unstained slides and 1 H&E slide to us at the following address. You will most likely be asked to fill out a form allowing them to send the slides to us.
