Hospital Admission Information
Dear Patient:
We are delighted that you have decided to deliver your baby at Sloane Hospital for Women at NewYork-Presbyterian Hospital. To facilitate your admission,we would greatly appreciate your completing the enclosed
pre-registration forms. Please return these forms in advance in the envelope provided.
When you arrive for your delivery, please go to:

Valet Parking is available.
You will be immediately directed to the Carmen and John Thain Labor Unit on the 10th floor. After 9 p.m., if you would like valet parking please ask your physician to call the Security Desk at (212) 305-2222 to alert them that you are on your way to the hospital. Also, please remember to call your insurance company to pre-certify your maternity admission. If you have more than one insurance company, please pre-certify with both of them. In addition, please be sure to include the name and phone number of the pediatrician you have selected to care for your child in the space provided on the registration form. We look forward to having you with us and we will do everything possible to make your stay as comfortable as possible.
