Wellness Checklist

Annual history and physical exam
Annual blood pressure, weight, and exam of head, eyes, ears, nose, throat, heart, lungs, abdomen, and extremities. Annual pelvic and rectal exam and Pap smear.
Breast self exam and mammogram
Monthly breast self-exam. Baseline mammogram between 35-40 years. Annual mammogram recommended for women >40 years.

Blood pressure
Measure annually.

HIGH BLOOD PRESSURE, or hypertension often has no symptoms; it is defined as > 140/90 mm Hg and requires treatment. PREHYPERTENSION is a blood pressure of 120/80-139/89 mm Hg and

NORMAL is less than 120/80 mm Hg

Baseline cholesterol (desirable: < 200 mg/dL; borderline high: 200-239 mg/dL; high: 240 mg/dL and above);

LDL, the "bad" cholesterol (optimal less than 100 mg/dL; near optimal 100-129mg/dL; high: 160-189 mg/dL); and

HDL, the "good" cholesterol (38-72 mg/dl). If normal, retest every 5 years.

A blood test for diabetes should be determined every 3 years.

Normal less than 99 mg/dL;

Prediabetes: 100-125 mg/dL;

Diabetes: 126 mg/dL and above.

Annual exam of skin. Change in shape, color, size or border of moles requires evaluation for skin cancer.
Colon Cancer Screening
Beginning age 50 annual stool blood testing, flexible sigmoidoscopy and/or colonoscopy.
Dental exams
Visit the dentist every 6 months. Three out of 4 people over 35 years have some form of gum or dental disease.
Eye Exam
Have an eye exam every 2 to 4 years, even if you do not have problems. Glaucoma (excessive pressure within the eye) often goes unnoticed and is a major cause of blindness.
Osteoporosis screening
Women aged > 65 years, postmenopausal women who have had a fracture, and postmenopausal women with a risk factor for osteoporosis.
Tetanus-diptheria boosters (every 10 years). Influenza vaccine for > 65 years, health care workers and those with chronic illness annually. Pneumoccal vaccine at age > 65 years (a one-time dose.) Women of childbearing age should have documented rubella immunization.

Visit nutritionist for healthy lifestyle. Increase physical activity. Smoking cessation. Exercise stress testing. Travel immunizations. BMI Normal weight: 18.5-24.9; Overweight: 25-29.9; Obese: 30 or greater
