Physicians Are Responsible for Service Claims Submitted for Payment Physicians are responsible for all billing submitted under their name. This includes the correct input of both the diagnoses and procedure codes. Physicians should be selecting the codes that best describe their services provided to patients. While many Certified Coders might be selecting the service codes, the ultimate responsibility for service related billing information remains with the physician.   Encounter forms, or charge tickets should be updated annually to ensure current accuracy for the service and diagnoses codes.   Return to Contents | Return to Billing Compliance
