A referral or authorization from your managed care plan may be required for your Consultation visit.

You may obtain this referral from your PCP (Primary Care Physician) or from the referring specialist (with some managed care plans). If you are not sure if a referral or authorization is required you must contact your plan directly. There is usually a toll free contact telephone number on your insurance card.

On the day of you appointment we ask that you bring the referral or authorization along with your insurance cards with you so we may make copies for our records.
